Medical & Other Forms
TPCA requires parents to complete and submit certain medical and information forms annually for each enrolled student. Questions about forms may be directed to the school nurse at jprice@tpcs.org. All required forms must be submitted prior to the first day of classes.
Medical Forms packet: Early Childhood - 12th Grade - includes annual physical and health history forms, CHIRP, OTC medication authorization among others
Dismissal Authorization Form - provides list of all individuals authorized to pick up a student
TPCA Technology Acceptable Use Policy Form - policy on how students may use TPCA technology requried annually for all students K - 12
